Dear Parents and Debutantes,

It is our pleasure to welcome you to participate in the 2014 Debutante Ball where you will become full members of the Ukrainian community. Below are event details for this year’s ball:

Fees: $450 for active members* / $550 for non-active members includes:

Dinner for debutante and escort
Flowers for debutante, escort, father and mother
Sash
One 8” x 10” group photo (by CYM-hired photographer)

REGISTRATION DEADLINE:	July 1, 2013 – registration form and $100 deposit
REGISTRATION AFTER JULY 1:	Fee for each debutante increases by $100.($550 for active members / $650 for non-active members)
FULL PAYMENT & BIOS DUE:	September 15, 2013

*To be considered an active member you must attend 50% of CYM events – schodyny, apeli, panachydy, other events, per attendance being taken by Bulava in 2013. The Bulavniy from your oseredok will have to sign the registration form enclosed.

The first meeting with the debutantes and parents is scheduled for Sunday, September 15 in Chicago CYM (2457 West Chicago Avenue) at 1:30 p.m. This is a mandatory meeting for both debutantes and parents.

Each debutante will need to submit a photograph in a Ukrainian embroidered blouse for the commemorative book. We will need a 5”x7” black & white photo (or jpeg file) sent to Kristin Chylak at [email protected] by October 1.

Debutantes will not be allowed to start practices until registration checks, photos and questionnaires are received.
