Camp Forms
Below is a list of forms (with links) which are required to be filled in by all campers and sent with camp payments.
You will see two tables below:
List of forms for those UNDER 18 years of age (Sumenyata & Yunatstvo);
List of forms for those 18 years and over (Druzhynnyky & Seniors).
All camp forms can be:
- Emailed to the UYA National Executive

- Sent to Stepan Duma at 230 Macquarie Road, Greystanes NSW 2145.
